    胶原透明质酸敷料用于痤疮凹陷性瘢痕点阵铒激光术后的临床观察

    New collagen and hyaluronic acid dressing for the treatment of atrophic facial acne scars combined with Erbium fractionated laser: a clinical observational study

    • 摘要: 目的 观察胶原透明质酸敷料用于铒激光点阵治疗痤疮凹陷性瘢痕术后的修复效果。方法 从门诊选择46例痤 疮凹陷性瘢痕患者,随机分为实验组和对照组,各23例。实验组在铒激光点阵治疗后外用胶原透明质酸敷料20 min ,共7 d ,对照组用纱布湿敷,其余治疗相同。治疗后随访3个月,观察术后炎症反应情况、结痂脱落时间、患者舒适度评价、VISIA皮肤检测仪评价红色区域、疗程后色素沉着率、渗出性创面恢复情况及其他不良反应的发生情况。结果:经卡方检验,实验组急性炎症反应评分优于对照组(2=9.544,P<0.05),实验组术后自我舒适程度评价优于对照组(2=7.413,P<0.05),实验组色素沉着发生率亦低于对照组(2=4.212,P<0.05);经t检验,实验组与对照组结痂脱落时间(t=5.338,P<0.001),VISIA下红色区域评分(t=4.553,P<0.001),实验组均低于对照组。结论:新型胶原透明质酸敷料在铒激光点阵治疗痤疮凹陷性瘢痕的术后应用可加快激光术后红斑状况恢复,缩短创面愈合、结痂脱落时间,提高术后舒适度。

       

      Abstract: ob<x>jective to evaluate the efficacy of a new collagen and hyaluronic acid dressing for healing of wounds induced by erbium fractionated laser treatment of atrophic scar in facial acne. Methods 46 patients from the outpatient department were recruited and randomly divided into treatment and control groups (23 cases in each group) . Both groups were then treated by two sessions of Erbium fractionated laser with an interval of one month. After each session of laser therapy, the treatment group were topically treated with a collagen and hyaluronic acid dressing for 20 minutes once a day during a 7 -day session, while a Saline-soaked gauze was applied to the control group. The 46 patients were followed up for 3 months after treatment to moniter the acute inflammatory response, scab sheddig time and patient comfort level. The incidence of post-inflammatory hyprepigmentation, the red areas assessment with VISIA skin tester, and other adverse reaction were also assessed. Results There were significant differences between treatment and control groups in acute inflammatory response score (2=9.544, P<0.05), patient comfort level after laser surgery (2=7.413, P<0.05) and incidence of hyprepigmentation (2=4.212, P<0.05) by Chi-square test. The scab shedding time(t =5.338, P<0.001) of treatment group has shorten and the red area score under VISIA (t=4.553,P<0.001)was significantly lower than that of the control group.Conclusion The application of a new collagen and hyaluronic acid dressing with tranexamic acid, dipotassium glycyrrhizinate after Erbium fractionated laser treatment can reduce both the incidence of adverse reactions and the time of scab shedding, but accelerate the recovery of skin erythema after Erbium fractionated laser therapy
