    帕金森病患者视网膜浅层血管密度及内层结构的定量分析及临床意义

    Quantitative analysis of the inner retina microvascular and structural changes in Parkinson disease patients and their clinical significance

    • 摘要: 目的 评估光学相干断层扫描血管成像(OCTA)及光学相干断层扫描(OCT)下帕金森病(PD)患者视网膜黄斑区浅层血管密度、视网膜内层厚度的变化特点,探讨其对PD的诊断价值。方法 选取2022年1月-2022年12月于徐州医科大学附属医院神经内科确诊的原发性PD患者50例作为PD组,并收集同期来院体检的健康中老年人56例作为对照组。比较2组间视网膜黄斑区浅层血管密度及视网膜内层厚度的变化特点,比较PD患者运动症状较重侧眼和较轻侧眼的差异。绘制受试者工作特征(ROC)曲线分析视网膜参数对PD的诊断价值。结果 PD组黄斑区各象限的浅层血管密度和视网膜内层厚度均小于对照组,差异有统计学意义(均P<0.05)。运动症状较重侧眼和较轻侧眼的视网膜浅层血管密度和视网膜内层厚度差异无统计学意义(均P>0.05)。ROC分析结果显示旁中心凹颞侧象限的视网膜内层厚度曲线下面积最大,截断值为129.5 μm, 敏感度为96.0%,特异度为82.1%。结论 PD患者视网膜黄斑区浅层血管密度降低,视网膜内层厚度萎缩变薄,在视网膜中对称发生。PD患者的视网膜改变可为PD的诊断提供一定的参考价值。

       

      Abstract: Objective To investigate the vessel density of the macular superficial capillary plexus (SCP) and the thicknesses of the inner retina in patients with Parkinson disease (PD) through optical coherence tomography angiography (OCTA) and optical coherence tomography (OCT).Methods A total of 50 patients with primary PD, who were admitted to Department of Neurology, the Affiliated Hospital of Xuzhou Medical University from January 2022 to December 2022 were selected as a PD group. Meanwhile, another 56 healthy subjects who underwent physical examination were enrolled as a control group. Both groups were compared for the changes in the vessel density of SCP and the thicknesses of the inner retina. For PD patients, the difference between the eye with severer motor symptoms and the other side was compared. A receiver operating characteristic curve (ROC) was plotted to evaluate the value of inner retina parameters for the diagnosis for PD.Results The PD group presented significant decreases in the density of SCP and the thickness of inner retina, compared with the control group (all P<0.05).No statistical difference was found as to the changes in the vessel density of SCP and the thicknesses of the inner retina in the eye with severer motor symptoms and the other side (all P>0.05). According to ROC analysis, the area under the curve (AUC) of the thickness of the inner retina in the parafoveal temporal quadrant was the largest, and the optimal cutoff value was 129.5 μm, with a sensitivity of 96.0% and a specificity of 82.1%.Conclusions Reduced vascular density of SCP and thinning of the inner retina can be observed in PD patients bilaterally, which provide certain reference value for PD diagnosis.
