    康复新液联合阿奇霉素对慢性鼻窦炎伴鼻息肉患者术后鼻腔功能的影响

    Effect of Kangfuxin liquid combined with azithromycin on postoperative nasal function in patients with chronic rhinosinusitis with nasal polyps

    • 摘要: 目的 探究康复新液联合阿奇霉素对慢性鼻窦炎伴鼻息肉(CRSwNP)患者术后鼻腔通气功能、嗅觉功能的影响。方法 选取118例在北京航天总医院进行功能性鼻内镜手术治疗的CRSwNP患者,分为对照组(59例)和联合组(59例)。其中对照组给予阿奇霉素治疗,联合组给予康复新液联合阿奇霉素治疗。比较2组临床疗效、鼻腔通气功能〔鼻腔最小横截面积(NMCA)、鼻腔容积(NCV)、鼻腔最小横截面积至前鼻孔距离(DCAN)〕、嗅觉功能(T&T标准嗅觉测试法)、鼻黏膜炎症指标〔白细胞介素-6(IL-6)、黏蛋白MUC5AC〕、鼻黏液纤毛清除功能(黏液纤毛清除率、黏液纤毛清除速度、糖精清除时间)及不良反应。结果 联合组总有效率(94.92%)明显高于对照组(81.36%)(P<0.05)。与治疗前比较,2组治疗后NCV、NMCA、黏液纤毛清除速度均明显升高(P<0.05),且与对照组比较,联合组明显升高(P<0.05)。2组治疗后DCAN、嗅觉功能评分、黏液纤毛清除率、糖精清除时间、MUC5AC、IL-6水平均明显降低(P<0.05),且与对照组比较,联合组明显降低(P<0.05)。2组均未出现肝肾功能异常、恶心及呕吐等不良反应。结论 康复新液联合阿奇霉素治疗CRSwNP患者可有效提高鼻纤毛清除功能、鼻腔通气功能、嗅觉功能,减轻局部炎症反应,安全有效。

       

      Abstract: Objective To explore the effect of Kangfuxin liquid combined with azithromycin on postoperative nasal ventilation function and olfactory function in patients with chronic rhinosinusitis with nasal polyps (CRSwNP).Methods A total of 118 patients with CRSwNP who underwent functional endoscopic sinus surgery in Beijing Aerospace General Hospital were selected. They were divided into two groups (n=59):a control group and a combined group. The control group was treated with azithromycin, while the combined group was given Kangfuxin liquid combined with azithromycin. Both groups were compared for clinical efficacy, nasal ventilation function nasal minimal cross-sectional area (NMCA), nasal cavity volume (NCV), and distance of the minimal cross-sectional area to the nostril (DCAN), olfactory function (T&T standard olfactory test method), nasal cavity mucosal inflammation indicators interleukin-6 (IL-6), and mucin MUC5AC, nasal mucociliary clearance function (mucociliary clearance rate, mucociliary clearance speed, and saccharin clearance time) and adverse reactions.Results The combined group showed a higher total effective rate (94.92%) than the control group (81.36%) (P<0.05). Compared with those before treatment, both groups showed remarkable increases in NCV, NMCA, and mucociliary clearance rates after treatment (P<0.05), where significant increases were seen in the combined group, compared with those in the control group (P<0.05). After treatment, both groups presented remarkable decreases in DCAN, post-olfactory function score, mucociliary clearance rate, saccharin clearance time, MUC5AC, and IL-6 levels (P<0.05), where significant decreases were seen in the combined group, compared with those in the control group (P<0.05). There were no adverse reactions such as abnormal liver and kidney function, vomiting and nausea in both groups.Conclusions Kangfuxin liquid combined with azithromycin is safe and effective for the treatment of CRSwNP patients, which can effectively improve nasal ciliary clearance function, nasal ventilation function and olfactory function, reduce local inflammatory response.
