    肿瘤坏死因子-a、核因子-κB在布加综合症肝纤维化中的表达研究

    Expression of tumor necrosis factor-a and nuclear factor-κB in liver fibrosis of Budd-Chiari syndrome

    • 摘要: 目的 通过检测布加综合征(BCS)患者血清中肿瘤坏死因子-α(TNF-α)、核因子-κB(NF-κB)的含量,探讨其在BCS发展至淤血性肝硬化乃至肝癌(HCC)中的作用。方法 实验组选取103例BCS患者,对照组选取同期健康体检130例。采用酶联免疫法(ELISA)检测两组人群血清中两因子含量。结果 实验组血清两因子含量均高于对照组,差异有统计学意义(P<0.001),实验组单纯BCS、BCS合并淤血性肝硬化、BCS合并肝癌患者血清两因子表达水平比较,差异有统计学意义( P<0.001)。BCS患者血清TNF-α与NF-κB呈正相关(r=0.8,P<0.001)。结论 在单纯BCS发展至淤血性肝硬化、肝癌进程中,TNF-α和NF-κB表达水平增加,早期介入治疗和抗纤维化治疗结合,对于减少淤血性肝硬化甚至肝癌的发生至关重要。

       

      Abstract: Objective To study the levels of tumor necrosis factor-α (TNF-α) and nucear factor-κB (NF-κB) in serum of patients with Budd-Chiari syndrome (BCS), and to explore whether serum TNF-α and NF-κB play an important roles in the process of simple BCS into congestive cirrhosis and liver cancer. Methods There were 103 cases of Budd-Chiari syndrome in the experimental group. During the same period, 130 controls were recruited from the health examination center. The levels of serum TNF-α and NF-κB were tested by ELISA. Results The serum concentrations of TNF-α and NF-κB in the experimental group were higher than those in the control group( P<0.001). In the BCS group, the serum concentrations of TNF-α and NF-κB were different significantly between pure BCS patients, BCS with stasis cirrhosis patients and BCS with hepatic carcinoma patients( P<0.001). Among them, the serum concentrations of the two factors of BCS with hepatic carcinoma patients were higher than BCS with stasis cirrhosis patients, and the latter was higher than the pure BCS patients( P<0.05). Serum TNF-α and NF-κB were positively correlated in the BCS group (r = 0.8, P <0.001). Conclusion The expression level of TNF-α and NF-κB increase in the process of pure BCS deteriorating into congestive liver cirrhosis and even liver cancer. The combination of interventional therapy and anti-fibrosis therapy at an early stage may reduce the incidence of congestive liver cirrhosis and even liver cancer.
