    彩色多普勒超声对视网膜脱离后眼血流动力学变化的研究

    A study on the changes of ocular hemodynamics after retinal detachment by color Doppler ultrasonography

    • 摘要: 目的应用彩色多普勒超声观察视网膜脱离(RD)后眼血流动力学的变化。方法选取于本院行彩色多普勒超声检查的45例原发性RD患者(45只患眼)作为RD组,将患者对侧健眼作为对照组,对2组眼动脉(OA)、视网膜中央动脉(CRA)、睫状后短动脉(PCA)的收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(RI)进行比较。对CRA和PCA的血流参数变化与患者的年龄、RD时间和范围进行相关分析。结果RD组OA的PSV、EDV、RI与对照组相比,差异无统计学意义(P>0.05)。RD组CRA和PCA的PSV、EDV、RI与对照组相比,差异有统计学意义(P<0.05)。视网膜脱离后RD时间与CRA的PSV变化、EDV变化及RI变化均有相关性(P<0.05),与PCA的PSV变化和EDV变化有相关性(P<0.05)。RD范围与CRA的EDV变化和RI变化有相关性(P<0.05)。年龄与PCA的EDV变化和RI变化有相关性(P<0.05)。结论RD可以降低球后眼血流速度,增加CRA与PCA的血流阻力。RD后球后血管血流动力学变化与年龄、RD的时间与范围等均有相关性。

       

      Abstract: ObjectiveTo investigate the changes of ocular hemodynamics after retinal detachment (RD) by color Doppler ultrasonography. MethodsA total of 45 patients (with 45 diseased eyes) who were admitted into our hospital due to primary retinal detachment (RD) and underwent color Doppler ultrasonography were set as an RD group, while their healthy eyes were set as controls. Both groups were compared for the peak systolic velocity (PSV), end diastolic velocity (EDV) and resistance index (RI) of the ocular artery (OA), the retinal central artery (CRA) and the posterior ciliary artery(PCA) blood flow. The association between the changes of blood flow parameters in CRA and PCA and the age, the time and range of RD were analyzed. ResultsCompared with the control group, the RD group showed no significantly different PSV, EDV and RI in OA (P>0.05), but remarkable different PSV, EDV and RI in CRA and PCA (P<0.05).The time of RD was significantly associated with the changes of PSV, EDV and RI in CRA (P<0.05) as well as the changes of PSV and EDV in PCA (P<0.05). The range of RD was associated with the changes of EDV and RI in CRA (P<0.05). The changes of EDV and RI in PCA were correlated to age (P<0.05). ConclusionsRD can reduce the retrobulbar blood flow velocity, increase the blood flow resistance of CRA and PCA. The age, time and range of RD are associated with hemodynamic changes of retrobulbar blood vessels.
