    LAMP法检测下呼吸道感染与肺癌合并下呼吸道感染病原体特征

    Pathogen characteristics of lower respiratory tract infection patients and lung cancer patients with lower respiratory tract infection detected by LAMP assay kit

    • 摘要: 目的 分析下呼吸道感染患者以及肺癌合并下呼吸道感染患者的痰液病原体特征,明确临床下呼吸道感染病原体,从而提升临床治疗效果.方法 收集于徐州市中心医院就诊的下呼吸道感染患者的痰液样本630例,使用环介导等温扩增(LAMP)十三联检试剂盒进行检测,分析检出的病原体特征.结果 下呼吸道感染患者的痰液样本中,阳性率最高的2种病原体是肺炎克雷伯菌(Kpn)和流感嗜血杆菌(Hin).与非肿瘤下呼吸道感染患者、无基础疾病下呼吸道感染患者比较,肺癌合并下呼吸道感染患者的痰液样本中Hin阳性率更高(P<0.05).LAMP法与培养法检测的一致率达68.7%.结论 LAMP法用于下呼吸道感染患者时病原体阳性检出率高,其中肺癌合并下呼吸道感染患者具有很高的Hin阳性率,这将为个性化治疗下呼吸道感染以及肺癌合并下呼吸道感染提供重要的参考依据.

       

      Abstract: ob<x>jective To analyze the pathogen characteristics of lower respiratory tract infection and lung cancer with lower respiratory tract infection, and to provide data support for clinical prevention and personalized treatment of lower respiratory tract infection. Methods From December 2018 to May 2019, a total of 630 sputum samples from patients with lower respiratory tract infection were collected in Xuzhou Central Hospital. For each sample, thirteen common pathogens of lower respiratory infection were analyzed by a commercially available loop-mediated isothermal amplification (LAMP) assay kit. Results A total of 630 patients with lower respiratory infections were included in this study. Pathogen characteristics detected in this study showes that the two most frequently detected pathogens are Klebsiella Pneumoniae and Haemophilus influenza, in all patients and patients with lower respiratory tract infection without underlying disease. Lung cancer patients with lower respiratory tract infection have a higher detection rate of Haemophilus influenzae than lower respiratory tract infection patients without tumor and patients with lower respiratory tract infection without underlying disease (P=0.03). The agreement rate between the LAMP method and the culture method was 70%. These results would provide important support for prevention and personalized treatment of lower respiratory tract infection
