    鼻腔嗜酸性粒细胞对儿童支气管哮喘的诊断价值及其与疾病严重程度关系的研究

    • 摘要: 目的 探讨鼻腔嗜酸性粒细胞(EOS)对儿童支气管哮喘的诊断价值及其与疾病严重程度的关系.方法 选取2017年6月—2018年12月在徐州医科大学附属连云港医院接受治疗的支气管哮喘患儿120例作为观察组,根据患病程度将其分为间歇组53例,轻度组44例,中度组12例,重度组11例;同时招募60例健康儿童作为对照组.比较对照组及观察组各亚组间鼻腔EOS水平.探讨鼻腔EOS对儿童支气管哮喘的诊断价值并与诱导痰及外周血EOS的诊断价值进行比较,同时分析鼻腔EOS与儿童哮喘控制测试(C-ACT)评分、第1秒用力呼气量(FEV1)、血清IgE水平及呼出气一氧化氮(FeNO)的相关性.结果 观察组各亚组鼻腔EOS水平均显著高于对照组,且观察组各亚组间鼻腔EOS水平差异有统计学意义(P<0.05).鼻腔EOS诊断儿童支气管哮喘的敏感度为80.1%,特异度为81.8%,诊断价值优于诱导痰EOS及外周血EOS.鼻腔EOS与C-ACT评分、FEV1值呈负相关,与血清IgE水平及FeNO浓度呈正相关.结论 支气管哮喘患儿鼻腔EOS水平与疾病严重程度呈正相关,可作为临床有效的诊断指标.

       

      Abstract: ob<x>jective To investigate the diagnostic value of nasal eosinophil in children with bronchial asthma and its relationship with disease severity. Method 120children with bronchial asthma who were treated in our hospital from June 2017 to December 2018 were selected as observation group. According to the degree of illness, the observation group cases were divided into intermittent group of 53 cases, mild group of 44 cases, moderate group of 12 cases, and severe group of 11 cases. 60 healthy volunteer children were Recruited as a control group. Observation of nasal eosinophil levels between the control group and the observation group. To investigate the diagnostic value of nasal eosinophil in children with bronchial asthma and to compare the diagnostic value of induced eosinophil in peripheral blood and peripheral blood. Analysis of the relationship between nasal eosinophils and C-ACT score, FEV1, serum IgE and exhaled nitric oxide. Results The levels of nasal eosinophil in the subgroups of the observation group were significantly higher than those in the control group, and the levels of nasal eosinophil in the subgroups of the study group were statistically significant (P<0.05). The sensitivity of nasal eosinophil in the diagnosis of childhood bronchial asthma is 80.1%, and the specificity is 81.8%. The diagnostic value of nasal eosinophil is better than that of induced eosinophil and peripheral blood eosinophil. Nasal eosinophil were negatively correlated with C-ACT score and FEV1 value, and positively correlated with serum IgE levels and exhaled nitric oxide. Conclusion The level of nasal eosinophil in children with bronchial asthma is positively correlated with the severity of the disease and can be used as a clinically effective diagnostic indicator. Nasal eosinophil is simple to operate and highly feasible, and are worthy of promotion and application.
