    Periostin和Ki67在宫颈癌中的表达及其与预后的关系

    Expression of periostin and Ki67 and their relationship with the prognosis of cervical carcinoma

    • 摘要: 目的 研究骨特异性因子(Periostin)和增殖细胞相关抗原(Ki67)在宫颈癌中的表达,探讨两者分别与宫颈癌患者临床病理特征的关系,并分析可能影响宫颈癌患者预后的危险因素。方法 选取86例宫颈癌、30例正常宫颈组织,免疫组织化学SP法检测其中Periostin和Ki67蛋白的表达,分析二者的阳性表达与宫颈癌临床病理特征的关系、在宫颈癌组织中表达的相关性,并评估影响宫颈癌患者术后总生存时间的危险因素。结果 在86例宫颈癌组织中,Periostin和Ki67的阳性表达率较正常宫颈组织中均显著升高(P<0.05),Periostin的阳性表达与临床分期、分化程度、脉管浸润及淋巴结转移有关(P<0.05),Ki67的阳性表达与临床分期、淋巴结转移及分化程度有关(P<0.05),两者在宫颈癌中的表达呈正相关关系(γ=0.319,P=0.003)。Periostin阳性的宫颈癌患者的总生存期显著降低(41.037个月 vs 63.672个月,Log-rank检验,χ2=15.892,P=0.000),Periostin阳性表达为影响宫颈癌患者术后总生存期的独立危险因素(HR=3.916, 95% CI:1.022~15.006, P=0.046<0.05)。结论 Periostin和Ki67在宫颈癌中的异常表达可能协同促进宫颈癌细胞的浸润及转移。Periostin阳性表达可能提示宫颈癌预后不良。

       

      Abstract: Objective To analyze the expression of periostin and Ki67 in cervical carcinoma, evaluate their relationship with the clinical pathological characteristics, and analyze the risk factors that may influence the prognosis of cervical carcinoma patients. Methods A total of 86 cases of cervical carcinoma and 30 cases of normal cervical tissues were collected. The expression of periostin and Ki67 was detected by immunohistochemistry. The relationship between the positive expression of periostin and Ki67 and the clinical pathological characteristics in cervical carcinoma, as well as the relationship between their expressions in cervical carcinoma were analyzed. Then, the risk factors affecting the overall survival after surgery in cervical carcinoma patients were evaluated. Results In 86 cervical carcinoma tissues, the positive rates of periostin and Ki67 were significantly higher than those in normal cervical tissues (P<0.05). The positive expression of periostin was correlated to clinical stage, tumor differentiation, vascular invasion and lymph node metastasis (P<0.05). Similarly, the positive expression of Ki67 was correlated to clinical stage, tumor differentiation, and lymph node metastasis (P<0.05), and there was a positive correlation between the two indexes in cervical carcinoma (γ=0.319, P=0.003). The overall survival was significantly lower in cervical carcinoma patients with positive periostin expression (41.037 months vs 63.672 months, Log-rank test χ2=15.892, P=0.000). The positive expression of periostin was an independent prognostic factor for overall survival in cervical carcinoma patients (HR=3.916, 95% CI: 1.022-15.006, P=0.046<0.05). Conclusions The abnormal expression of periostin and Ki67 may synergistically stimulate the invasion and metastasis of cervical carcinoma. The positive expression of periostin may indicate a poor prognosis in cervical carcinoma.
