    袖状胃切除术对肥胖患者肺功能的影响

    Effects of sleeve gastrectomy on lung function in obese patients

    • 摘要: 目的 探讨袖状胃切除术减重后对肥胖患者肺功能的影响,并分析减重后体重指数(BMI)、颈围和腹围变化量与肺功能指标改善量之间的相关性。方法 选取2022年10月—2024年10月在徐州医科大学附属医院行袖状胃切除术的60例肥胖患者的临床资料。采用配对t检验或Wilcoxon配对符号秩检验比较减重手术前后患者的肺功能和症状评分等指标的变化。Spearman或Pearson相关性分析判定ΔBMI、Δ颈围、Δ腹围与肺功能改善量之间的相关性。结果 减重手术3个月后患者BMI、颈围、腹围明显下降,嗜睡、认知、焦虑及抑郁症状明显改善,差异有统计学意义(P<0.05)。减重手术后肺活量(VC)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、1秒率(FEV1/FVC)、呼气流量峰值(PEF)、最大呼气中期流量(MMEF)、肺总量(TLC)、肺一氧化碳弥散量(DLCO)较术前明显提升(P<0.05)。相关性分析显示ΔBMI和Δ腹围均与ΔVC、ΔFVC、ΔFEV1、ΔFEV1/FVC、ΔPEF、ΔMMEF、ΔTLC、ΔDLCO呈正相关(P<0.05);Δ颈围与ΔVC、ΔFVC、FEV1、ΔTLC呈正相关(P<0.05),与ΔFEV1/FVC、ΔPEF、ΔMMEF、ΔDLCO未见明显相关性(P>0.05)。结论 袖状胃切除术减重后有效改善肥胖患者肺通气与肺换气功能。

       

      Abstract: Objective To explore the impact of sleeve gastrectomy on lung function in obese patients, and to investigate the correlation between changes in body mass index (BMI), neck and abdominal circumference, and improvements in lung function indicators after weight loss.Methods A total of 60 obese patients who underwent sleeve gastrectomy at the Affiliated Hospital of Xuzhou Medical University from October 2022 to October 2024 were selected. The changes in lung function and symptom scores before and after weight loss surgery were compared by Paired t-tests or Wilcoxon signed-rank tests. Spearman or Pearson correlation analysis was conducted to determine the correlation between changes in BMI (ΔBMI), neck circumference (Δneck), abdominal circumference (Δabdominal), and improvements in lung function.Results Three months after weight loss surgery, the patients showed significant reductions in BMI, neck circumference, and abdominal circumference. Symptoms such as sleepiness, cognitive impairment, anxiety, and depression were significantly improved, with statistically significant differences (P<0.05). Lung function parameters, including vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1/FVC ratio, peak expiratory flow (PEF), maximal mid-expiratory flow (MMEF), total lung capacity (TLC), and diffusing capacity of the lung for carbon monoxide (DLCO), were significantly improved after surgery compared with preoperative values (P<0.05). Correlation analysis showed that ΔBMI and Δabdominal circumference were positively correlated with ΔVC, ΔFVC, ΔFEV1, ΔFEV1/FVC, ΔPEF, ΔMMEF, ΔTLC, and ΔDLCO (P<0.05). ΔNeck circumference was positively correlated with ΔVC, ΔFVC, FEV1, and ΔTLC (P<0.05), but no significant correlation was found with ΔFEV1/FVC, ΔPEF, ΔMMEF, and ΔDLCO (P>0.05).Conclusions Sleeve gastrectomy effectively improves lung ventilation and gas exchange function in obese patients.
