    儿童非典型皮损颜色幼年性黄色肉芽肿的超声图像与病理学分析

    Ultrasonic imaging and pathological analysis of juvenile xanthogranuloma with atypical skin lesion colors in children

    • 摘要: 目的 分析儿童非典型皮损颜色幼年性黄色肉芽肿(JXG)的超声图像特点,结合病理分期,提高对该病的认识。方法 选择2020年1月—2025年6月于徐州医科大学附属徐州儿童医院经组织病理学确诊的非典型皮损颜色JXG患儿28例,对其临床资料、病理学特征、超声征象进行回顾性分析。结果 28例患儿中,男性14例,女性14例,皮损颜色为红色12例(42.9%),青紫色2例(7.1%),肤色14例(50.0%)。病变位于真皮层及脂肪层,部分病变累及肌层浅层,超声表现为低回声15例(53.6%)和混合回声13例(46.4%);界限清晰23例(82.1%),界限欠清晰5例(17.9%);结节型表现为小结节型12例(42.9%),大结节型12例(42.9%),巨大结节型4例(14.2%)。Alder血流分级中,0级14例(50.0%),Ⅰ级7例(25.0%),Ⅱ级7例(25.0%)。病变分期中,早期病变10例(35.7%),成熟期病变13例(46.4%),消退期病变5例(17.9%)。根据组织病理学检查分期,各组间在性别和超声回声性质上的差异有统计学意义(P<0.05)。结论 儿童非典型皮损颜色JXG的超声表现与不同的组织病理分期密切相关,具有一定的特征性,这为临床诊疗和治疗提供了有价值的参考依据。

       

      Abstract: Objective To analyze the ultrasonic imaging characteristics of juvenile xanthogranuloma (JXG) in children with atypical skin lesion colors and discuss with considerations of pathological staging to enhance the understanding of this disease. Methods Retrospective analysis was conducted on 28 children who were diagnosed with atypical colored JXG through histopathological examination at Xuzhou Children's Hospital Affiliated to Xuzhou Medical University from January 2020 to July 2025. Their clinical data, pathological features, and ultrasound findings were analyzed. Results Among the 28 children, 14 were male and 14 were female. The lesion colors were red in 12 cases (42.9%), purple-blue in 2 cases (7.1%), and skin-colored in 14 cases (50.0%). The lesions were located in the dermis and subcutaneous fat layers, with some extending to the superficial muscle layer. Ultrasound findings showed hypoechoic in 15 cases (53.6%) and mixed echogenicity in 13 cases (46.4%). Clear boundaries were observed in 23 cases (82.1%) and unclear boundaries in 5 cases (17.9%). Nodule types included small nodules in 12 cases (42.9%), large nodules in 12 cases (42.9%), and giant nodules in 4 cases (14.2%). According to the Alder blood flow grading, grade 0 was observed in 14 cases (50.0%), grade Ⅰ in 7 cases (25.0%), and grade Ⅱ in 7 cases (25.0%). Staging of the lesions showed early stage in 10 cases (35.7%), mature stage in 13 cases (46.4%), and regressive stage in 5 cases (17.9%). Statistical analysis revealed significant differences in sex and ultrasonic echo characteristics across different pathological stages (P<0.05). Conclusions The ultrasound characteristics of pediatric atypical colored JXG are closely related to different pathological stages, with certain distinguishing features that provide valuable reference for clinical diagnosis and treatment.
