    卵巢型子宫内膜异位症患者组织及血液中CYR61、miR-22-3p的表达及临床意义

    Expression and clinical significance of CYR61 and miR-22-3p in tissues and blood of patients with ovarian endometriosis

    • 摘要: 目的 研究高半胱氨酸蛋白61(CYR61)与微小RNA-22-3p(miR-22-3p)在卵巢型子宫内膜异位症(EMs)中的表达及临床意义。方法 选取于2025年1月—8月于徐州医科大学附属医院经腹腔镜手术及病理学检查确诊的40例卵巢型EMs患者。收集其异位囊肿囊壁组织(异位组)、在位子宫内膜组织(在位组);选取同期40例因子宫平滑肌瘤行子宫切除术的患者,并收集其正常子宫内膜组织(对照组)。同时采集40例EMs患者术前静脉血作为研究组、40例因子宫肌瘤切除子宫的患者的术前静脉血作为对照组。采用免疫组化法检测组织CYR61蛋白表达,实时荧光定量PCR(qRT-PCR)检测组织与血液中miR-22-3p及组织中CYR61 mRNA表达水平,酶联免疫吸附(ELISA)法检测血液CYR61蛋白水平。分析CYR61、miR-22-3p、糖类抗原125(CA125)与临床病理特征的相关性,绘制受试者工作特征(ROC)曲线,分析各指标的诊断价值。结果 在组织样本中,异位组CYR61与miR-22-3p的表达阳性率及RNA表达水平均显著高于在位组和对照组(P<0.001);而在位组与对照组间差异无统计学意义(P>0.05)。异位组子宫内膜组织中CYR61蛋白的阳性表达率显著高于对照组及在位组(P<0.05),而在位组与对照组间差异无统计学意义(P>0.05)。在血液样本中,研究组血浆miR-22-3p及血清CYR61的表达水平显著高于对照组,差异具有统计学意义(P<0.001)。卵巢型EMs患者中,CYR61与miR-22-3p表达呈正相关(r=0.543,P<0.001)。ROC曲线分析显示,CYR61、miR-22-3p、CA125及三者联合诊断的曲线下面积(AUC)分别为0.830、0.895、0.919和0.990(AUC均>0.80)。三者联合诊断的效能优于任一指标单独诊断。结论 在EMs患者中,CYR61与miR-22-3p在血液及异位囊肿囊壁中的表达均显著升高,且两者表达水平呈正相关,提示它们可能参与了EMs的病理过程。CYR61、miR-22-3p与CA125的联合检测可能为子宫内膜异位症的临床诊断与病情随访提供新的潜在生物指标物,并为探索其治疗方案提示了新的治疗靶点。

       

      Abstract: Objective To investigate the expression of cysteine-rich protein 61 (CYR61) and microRNA-22-3p (miR-22-3p) in ovarian endometriosis (EMs) and their clinical significance.Methods A total of 40 ovarian EMs patients, who underwent laparoscopic surgery and pathologically diagnosed at the Affiliated Hospital of Xuzhou Medical University from January to August 2025, were selected. Ectopic cyst wall tissue (ectopic group) and normal endometrial tissue (in-situ group) were collected. Furthermore, 40 patients undergoing hysterectomy for uterine leiomyoma were selected as the control group, with normal endometrial tissue collected. Preoperative venous blood was collected from 40 EMs patients (study group) and 40 patients with uterine myoma (control group). CYR61 protein expression in tissues was detected by immunohistochemistry. MiR-22-3p and CYR61 mRNA expression in tissue and blood samples were examined by quantitative real-time PCR (qRT-PCR). The levels of CYR61 protein in blood samples were measured by enzyme-linked immunosorbent assay (ELISA). The correlations between CYR61, miR-22-3p, and CA125 with clinical and pathological features were analyzed. Receiver operating characteristic (ROC) curves were plotted to assess the diagnostic value of each indicator.Results In tissue samples, the positive expression rate and the levels of CYR61 mRNA and miR-22-3p in the ectopic group were significantly higher than those in the in-situ and control groups (P<0.001), with no significant difference between the in-situ group and the control group (P>0.05). The positive rate of CYR61 protein expression in the ectopic endometrial tissue was significantly higher than that in the in-situ and control groups (P<0.05), with no significant difference between the in-situ and control groups (P>0.05). In blood samples, the levels of plasma miR-22-3p and serum CYR61 levels in the study group were significantly higher than those in the control group (P<0.001). In ovarian EMs patients, CYR61 expression was positively correlated with miR-22-3p expression (r=0.543, P<0.001). ROC curve analysis revealed that the areas under the curve (AUC) for CYR61, miR-22-3p, CA125, and the combined diagnosis of the three were 0.830, 0.895, 0.919, and 0.990, respectively (all AUC >0.80). The diagnostic efficacy of the combined indicators was superior to that of any single indicator alone.Conclusions In EMs patients, the expression of CYR61 and miR-22-3p in blood and ectopic cyst wall tissue is significantly elevated, and their levels were positively correlated. These findings suggest that they may be involved in the pathological process of EMs. The combined detection of CYR61, miR-22-3p, and CA125 may provide new potential biomarkers for the clinical diagnosis and monitoring of endometriosis and offer new therapeutic targets for its treatment.
