    COVID-19患者T细胞计数的变化及影响因素分析

    Analysis of changes and influencing factors of T cell count in COVID-19 patients

    • 摘要: 目的 通过检测分析2019冠状病毒病(coronavirus disease 2019, COVID-19)患者CD4+T细胞计数的变化及影响因素,探讨外周血CD4+T细胞计数和CD4+T/CD8+T比值在COVID-19患者初期诊断及治疗评估中的应用价值。方法 选择2020年1月20日—2020年3月4日阜阳市第二人民医院进行首次辅助性T细胞检测的COVID-19确诊患者共101例,回顾性分析普通型(n=81)和重症型(n=20)患者首次外周血CD4+T细胞计数和CD4+T/CD8+T比值差异,分析性别、年龄、病情轻重、基础疾病等对患者T细胞计数变化的影响。结果 重症型病例CD4+T细胞计数明显低于普通型病例〔(315.35±50.56)个/μl vs.(467.00±21.38)个/μl〕,差异有统计学意义(P<0.05);CD4+T/CD8+T比值差异无统计学意义(P>0.05)。与COVID-19患者CD4+T细胞计数相关的因素是年龄、CD4+T/CD8+T细胞比值和诊断分型。结论 COVID-19患者的免疫功能受到影响,重症型患者外周血CD4+T细胞计数低于普通型患者。对于年龄>50岁以及合并基础性疾病患者的免疫功能应予以关注。

       

      Abstract: Objective To explore the application value of CD4+ T cell count and CD4+ T/CD8+ T ratio in the early diagnosis and treatment evaluation of patients with coronavirus disease 2019 (COVID-19). Methods From January 20, 2020 to March 4, 2020, 101 patients diagnosed with COVID-19 in Fuyang Second People's Hospital were selected as the research subjects. The differences of first time CD4+ T cell count and CD4+ T/CD8+ T cell ratio in peripheral blood between common patients (n=81) and severe-critical patients (n=20) were analyzed retrospectively, and the effects of gender, age, disease severity and underlying medical conditions on T cell count were analyzed. Results The first time count of CD4+ T cells in the severe-critical patients was significantly lower than that of the common patients (315.35±50.56)/μl vs. (467.00±21.38) /μl), the difference was statistically significant (P<0.05). There was no significant difference in CD4+ T/CD8+ T cell ratio between the two groups (P>0.05). The factors influencing CD4+ T cell count were age, CD4+ T/CD8+ T cell ratio and clinical classification of COVID-19. Conclusions The immune function of patients with COVID-19 is affected. The decrease of CD4+ T cell count in severe-critical patients is more serious than that in common patients. Attention should be paid to the immune function of patients aged older than 50 years and those with underlying medical conditions.
