    广泛性焦虑症患者心率变异性时域参数临床观察

    Clinical observation on time domain parameters of heart rate variability in patients with generalized anxiety disorder

    • 摘要: 目的 通过观察广泛性焦虑症(generalized anxiety disorder,GAD)患者心率变异性(heart rate variability,HRV)时域参数的变化,试图寻找出可靠的生理性量化指标。方法 选取2017年6月—2019年10月在江苏省中医院就诊的可疑GAD患者,采用贝克焦虑量表(Beck Anxiety Inventory,BAI)进行测评,筛选出56例评分≥45分的GAD患者,同时招募52例量表评分<45分的健康人群作为对照组。2组人群均行动态心电图(ambulatory electrocardiogram,AECG)检查,测定5项HRV时域参数并分别进行比较。其中相邻R-R 间期之差>50 ms的心搏数占总心搏数的百分比(pNN50)、全程相邻R-R 间期差值的均方根(rMSSD)反映副交感神经的功能;全程每5 min R-R间期标准差的均值(SDNN-Index)反映交感神经张力变化;全程全部R-R 间期标准差(SDNN)反映交感神经整体调节能力;全程R-R间期总数与最大R-R间期频度的比值(三角指数)则表达了24 h内所有的HRV。结果 与对照组相比,观察组pNN50、rMSSD和SDNN-Index显著下降(P<0.05),而SDNN和三角指数差异无统计学意义(P>0.05)。结论 GAD患者表现为副交感神经功能下降,交感神经活性增强。

       

      Abstract: Objective By observing the changes of time domain parameters of heart rate variability (HRV) in patients with generalized anxiety disorder (GAD), we tried to find a reliable physiological quantitative index. Methods From June 2017 to October 2019, suspicious GAD patients in Jiangsu Province Hospital of Chinese Medicine were selected. Beck Anxiety Inventory (BAI) was used to select 56 GAD patients with BAI score≥45 as observation group and 52 healthy people with BAI score <45 were recruited as control group. Ambulatory electrocardiogram (AECG) was performed in both groups and 5 HRV time domain parameters were measured and compared. The function of parasympathetic nerve was reflected by the percentage of the number of heart beats whose R-R interval difference is more than 50 ms to the total number of heart beats (pNN50) and the root mean square of the difference between adjacent R-R intervals (rMSSD). The mean value of standard deviation of R-R interval every 5 min (SDNN index) reflected the change of sympathetic tension .The standard deviation of all R-R interval (SDNN) reflected the whole regulation ability of autonomic nerve. The ratio of the total R-R interval to the maximum R-R interval frequency (trigonometric index) expressed all HRV within 24 h. Results Compared with the control group, pNN50,rMSSD and SDNN-Index in the observation group were significantly decreased (P<0.05), but there was no significant difference in SDNN and trigonometric index between the two groups (P>0.05). Conclusions GAD patients are characterized as decreased parasympathetic function and increased sympathetic activity.
