    NF-kB p50和NF-kB p65在胃癌组织中的表达及临床意义

    Expression and clinical significance of NF-kB p50 and NF-kB p65 protein in gastric carcinoma using tissue microarray

    • 摘要: 目的 探讨NF-kB p50及NF-kB p65在胃癌组织中的表达及其与胃癌患者各临床病理参数之间的关系并探讨其与胃癌患者预后的关系。方法 建立组织芯片,通过免疫组织化学SP法检测129例无术前放、化疗史的胃癌患者手术标本和101例癌旁组织中NF-kB p50及NF-kB p65的表达情况,并对随访患者进行生存分析。结果 NF-kB p50在胃癌组织中阳性表达率为84.5%,在癌旁组织表达率为32.7%,2组差异有统计学意义(P<0.01); NF-kB p50阳性表达与肿瘤TNM分期、分化程度和淋巴结转移有关(P<0.05)。NF-kB p65在胃癌组织中阳性表达率为79.1%,在癌旁组织表达率为27.7%,2组差异有统计学意义(P<0.01);NF-kB p65阳性表达与肿瘤TNM分期和淋巴结转移有关(P<0.05)。Kaplan-Meier生存分析结果显示,NF-kB p50和NF-kB p65阳性表达均与胃癌不良预后有关(P值均<0.01),并且通过联合检测结果显示,NF-kB p50-/NF-kB p65-表达组和NF-kB p50±/NF-kB p65?表达组的生存时间均比NF-kB p50+/NF-kB p65+表达组的生存时间长(P<0.01)。经Cox比例风险回归模型分析显示,NF-kB p50、NF-kB p65、肿瘤TNM临床分期和淋巴结转移与患者预后有关,且NF-kB p50可作为独立的预后因素。结论 NF-kB p50和NF-kB p65在胃癌组织中均高表达,NF-kB p50阳性表达与肿瘤TNM分期、分化程度和淋巴结转移有关;NF-kB p65阳性表达与肿瘤TNM分期和淋巴结转移有关。NF-kB p50和NF-kB p65阳性表达均与胃癌患者不良预后相关,且NF-kB p50可作为胃癌患者独立的预后因素。同时联合检测发现NF-kB p50+/NF-kB p65+表达组患者的生存时间最短,因此联合检测NF-kB p50和NF-kB p65可能有助于胃癌的早期诊断及预后分析,并进一步提示NF-kB p50和NF-kB p65可能在胃癌发生、发展中起重要作用。

       

      Abstract: Objective To investigate the expression and significance of NF-kB p50 and NF-kB p65 protein in gastric carcinoma and normal tissues adjacent to carcinoma and to evaluate the relationship between the expressions and the clinical pathologic factors. Methods High throughput technology of tissue microarray and immunohistochemical SP method were applied to detect the expressions of NF-kB p50 and NF-kB p65 protein in129 gastric carcinoma specimens that did not have a history of chemo-radiotherapy and 101 non-tumor gastric tissues. The followed-up cases were used for prognosis analysis. Results In gastric carcinoma tissue and normal gastric tissue, the rates of the expression of NF-kB p50 protein were 84.5%and 32.7% respectively(P<0.05).High NF-kB p50 expression related to tumor TNM stage, differentiation and lymph node metastasis ( P <0.05). The rates of the expressions of NF- kB p65 protein were 79.1% in gastric carcinoma tissue while 27.7% in normal tissues adjacent to carcinoma. NF-kB p65 protein showed relationship with tumor TNM stage and lymph node metastasis (P<0.05). Patients with negative expression for NF-kB p50 and NF-kB p65 had much higher survival rates than those with positive expression (all P<0.01). Cox proportional hazard regression model analysis showed that NF-kB p50 and TNM stage were associated with the prognosis of gastric carcinoma patients, and could be used as an independent prognostic factor. Conclusions In the evolution of gastric carcinoma, the over expression of NF-kB p50 and NF-kB p65 expression may cause abnormal cell proliferation and differentiation, leading to the occurrence and development of tumor
