    依达拉奉预处理对单肺通气患者HGMB1的影响

    Effects of edaravone preconditioning on HGMB1 in patients with one lung ventilation

    • 摘要: 目的 评估依达拉奉预处理对 单肺通气(OLV)患者高迁移率族蛋白B1(HMGB1)的影响。方法 选择2016年1月 ~2017年1月徐州医科大学附属医院需OLV患者50例,年龄在62~75岁之间,ASA分级Ⅰ~Ⅱ级,按随机数字表法分为2组(n=25),对照组(生理盐水组)、观察组(依达拉奉组),每组25例。观察组麻醉前微量泵静脉注射依达拉奉5mg/kg,30分钟内注射完成,对照组注射相同容积生理盐水。两组分别在依达拉奉注射前(T1)、OLV 3h(T2)、术后24h(T3)、术后48h(T4)测HMGB1和记录SaO2;记录麻醉后OLV前(T1)、OLV3h时患者PETCO2、气道平台压(Ppean)、气道峰压(Ppeak);记录术后48h内发生ALI/ARDS例数。结果 观察组T 2-4时HMGB1、SaO2均高于对照组;观察组T2时PETCO2、Ppean、Ppeak低于对照组;观察组术后48h内发生ALI/ARDS例数低于对照组,P均<0.05,有统计学意义。结论 依达拉奉可以通过降低 HMGB1表达减少肺损伤,保护肺功能。

       

      Abstract: Objective To evaluate the effects of edaravone precondi- tioning on high-mobility group box 1 in patients with one lung venti-lation. Methods Fifty ASA physical status Ⅰ~Ⅱ patients of both sexes ,aged 66—75 yr from January 2016 to January 2017 at Affiliated Hospital of Xuzhou Medical University,undergoing elective OLV, were randomly divid ed into two groups (n=25 each) : control group (saline group) and observation group (edaravone preconditioning group). In observation group edaravone 5mg/kg was infused intravenously for 30 min, while in control group the equal volume of normalsaline was given instead.Edaravone b efore injection(T1)、at 3h of OLV(T2)、at 24h of OLV(T3)、at 48h after operation (T4),venous blood samples were obtained for determination of the serum high-mobility group box 1 were recorded , SaO2 too.Before OLV (T1)、at 3h of OLV(T2)Pmean、Ppeak、PETCO2 were recorded.The number of ALI/ARDS at 48h after operation were recorded. Results HMGB1 of two groups at T2-4 were higher than the same group T 1, SaO2 was lower;The HMG B1 and SaO 2 of group B were higher than group A at T 2-4 ( P<0.05); The Ppean、Ppeak、 PETCO2 of two groups at T2 were higher than the same group at T1; The Ppean、Ppeak、 PETCO2 of group B were lower than group A at T 2(P>0.05); Incidence of ALI/ARDS of group B were lower than group A at 48h after operation . Conclusio n Edaravone may reduce lung injury by reduced expression of HMGB1 on the pulmonary function
