    血浆ET-1、D-D、sVCAM-1、ICAM-1水平预测突发性耳聋患者预后的价值

    Prognostic application of plasma ET-1, D-D, sVCAM-1 and ICAM-1 in patients with sudden deafness

    • 摘要: 目的 探讨血浆内皮素-1(ET-1)、D-二聚体(D-D)、血浆可溶性血管细胞黏附分子-1(sVCAM-1)、细胞间黏附因子-1(ICAM-1)在预测突发性耳聋患者预后方面的价值。方法 选取2020年6月-2022年6月上海市杨浦区控江医院收治的老年突发性耳聋患者130例,作为观察组。选取同期检查的健康体检者120例,作为对照组。采用酶联免疫法(ELISA)测量2组患者血浆ET-1、D-D、sVCAM-1、ICAM-1水平。采用受试者工作特征(ROC)曲线评估血浆ET-1、D-D、sVCAM-1、ICAM-1水平预测突发性耳聋患者预后的价值。结果 观察组血浆ET-1、D-D、sVCAM-1、ICAM-1水平显著高于对照组,差异有统计学意义(P<0.05)。与高频突发性耳聋相比,低频突发性耳聋患者血浆ET-1、D-D、sVCAM-1、ICAM-1水平更高,差异有统计学意义(P<0.05)。随着突发性耳聋患者听力损失的加重,血浆ET-1、D-D、sVCAM-1、ICAM-1水平显著升高,差异有统计学意义(P<0.05)。与治疗有效组相比,治疗无效组血浆ET-1、D-D、sVCAM-1、ICAM-1水平显著提升,差异有统计学意义(P<0.05)。血浆ET-1、D-D、sVCAM-1和ICAM-1联合应用时预测治疗有效的灵敏度、特异度高于单一指标预测(P<0.05)。结论 血浆ET-1、D-D、sVCAM-1、ICAM-1水平升高与突发性耳聋的发生及预后有一定的关系。上述治疗联合检测将有助于临床更好地评价突发性耳聋患者的预后。

       

      Abstract: Objective To evaluate the application of plasma endothelin-1 (ET-1), D-dimer (D-D), plasma soluble vascular cells adhesion molecule-1 (sVCAM-1) and intercellular adhesion factor -1 (ICAM-1) in predicting the prognosis of patients with sudden deafness.Methods A total of 130 patients with sudden deafness who were admitted to Control River Hospital of Yangpu District from June 2020 to June 2022 were selected as an observation group. Meanwhile, another 120 healthy subjects who underwent physical examination were selected as a control group. Their plasma levels of ET-1, D-D, sVCAM-1 and ICAM-1 were measured by enzyme-linked immunoassay (ELISA). The value of plasma ET-1, D-D, sVCAM-1 and ICAM-1 in predicting the prognosis of these patients with sudden deafness was evaluated by receiver specific curve (ROC) analysis.Results The plasma levels of ET-1, D-D, sVCAM-1 and ICAM-1 in the observation group were significantly higher than those in the control group (P<0.05). The levels of ET-1, D-D, sVCAM-1, and ICAM-1 in patients with low-frequency sudden deafness were remarkably higher than those with high-frequency sudden deafness (P<0.05). The plasma levels of ET-1, D-D, sVCAM-1 and ICAM-1 significantly increased with the aggravation of hearing loss in patients with sudden deafness (P<0.05). Compared with the effective group, the ineffective group presented remarkably increased levels of ET-1, D-D, sVCAM-1 and ICAM-1 (P<0.05). For prognosis prediction, the sensitivity and specificity of ET-1, D-D, sVCAM-1 and ICAM-1 in combination were higher than those of each single index (P<0.05).Conclusions The increase of plasma ET-1, D-D, sVCAM-1 and ICAM-1 levels is related to the occurrence and prognosis of sudden deafness. The combined detection of the above indicators will facilitate better evaluation of the prognosis of patients with sudden deafness in clinical practice.
