    1 194例特应性皮炎患儿血清过敏原检测及分析

    Detection and analysis of serum allergens in 1 194 children with atopic dermatitis

    • 摘要: 目的 分析徐州及其周边地区特应性皮炎(AD)患儿血清过敏原分布特征,为该地区AD的预防与治疗提供参考依据。方法 收集1 194例AD患儿临床资料,并对血清总IgE(tIgE)和过敏原特异性IgE(sIgE)检测结果进行统计分析。结果 1 194例AD患儿中,tIgE阳性率75.63%,sIgE阳性率60.72%,其中1项sIgE阳性者占18.01%,2项及以上阳性者占42.71%。食入性过敏原中牛奶阳性率(34.59%)最高,腰果(12.48%)次之,鸡蛋白(10.97%)位列第3;吸入性过敏原中阳性率前3位的分别是户尘螨(22.53%)、狗毛皮屑(18.76%)、猫毛皮屑(13.57%)。大多数过敏原(共14种)在不同年龄组间的差异有统计学意义(P<0.05),其中鸡蛋白、牛奶及腰果的阳性率随年龄增长而下降;户尘螨、屋尘、蟑螂、桑树以及柏榆柳栎等的阳性率随年龄增长而升高;猫毛皮屑及狗毛皮屑在<3岁组中高发,3岁后则明显降低。在>6岁组中,女童蟹阳性率明显高于男童(P<0.05),而男童腰果阳性率明显高于女童(P<0.05)。结论 徐州市及其周边地区大多数AD患儿属于IgE依赖型且以多项过敏原混合过敏多见,鸡蛋白、牛奶、腰果及户尘螨、狗毛皮屑、猫毛皮屑为主要过敏原。随年龄增长,AD患儿常见过敏原发生从食入性到吸入性的变化,提示年龄可能是AD的重要影响因素。

       

      Abstract: Objective To investigate the distribution characteristics of serum allergens of patients with atopic dermatitis (AD) in Xuzhou and its surrounding areas, so as to provide reference for its prevention and treatment in this region. Methods Clinical data were collected from 1 194 AD chidren and their results of serum total IgE (tIgE) and allergen-specific IgE (sIgE) were analyzed. Results Among 1 194 AD children, the tIgE positive rate was 75.63%, while the sIgE positive rate was 60.72%, in which 18.01% of sIgE positive carriers had one allergen and 40.71% of sIgE positive carriers had two and more allergens. The most common ingested allergen was milk (34.59%), followed by cashew nuts (12.48%) and egg albumen (10.97%). The top three inhaled allergens were house dust mite (22.53%), dog hair dander (18.76%) and cat hair dander (13.57%). There was statistical difference in most of allergens (14 types in total) among different age groups (P<0.05). With the increase of age, the positive rate of egg white, milk and cashew nuts decreased, while the positive rate of house dust mite, house dust, cockroach, mulberry and oak of cypress elm and willow increased. Cat hair dander and dog hair dander allergies were more common in the <3-year-old group, but less common after 3 years old. In the > 6-year-old group, the positive rate of crab in girls was significantly higher than that in boys (P<0.05), while the positive rate of cashew nuts in boys was significantly higher than that in girls (P<0.05). Conclusions Most AD children in Xuzhou and its surrounding areas are IgE-dependent and commonly allergic to multiple allergens, where the main allergens were egg albumen, milk, cashew nuts and inhaled house dust mite, dog hair dander and cat hair dander. With the increase of age, the common allergens in AD children changes from ingested allergens to inhaled ones, suggesting that age may play an important role in the development of AD.
