Pharmaceutical care for peritoneal dialysis of a patient with chronic kidney disease stage 5

Objective To provide pharmaceutical care for peritoneal dialysis patients, in order to promote the rational and effective clinical medication. Methods We participated in making the therapeutic plan of a patient with chronic kidney disease stage 5 for peritoneal dialysis and implemented pharmaceutical care according to the specific requirements of the patient. Results The pharmaceutical care implemented by clinical pharmacists was helpful to improve the effectiveness of drug treatment and medication compliance. Conclusions Clinical pharmacists should actively carry out pharmaceutical care and promote normalization and pattern of this work.
