Effects of laparoscopic total hysterectomy combined with uterosacral ligament shortening in treatment of patients with uterine prolapse in the perimenopausal period

Abstract
Objective To evaluate the efficacy of laparoscopic total hysterectomy combined with uterosacral ligament shortening in treatment of uterine prolapse patients in the perimenopausal period. Methods A total of 30 patients, aging 40-60, with uterine prolapse in the perimenopausal period underwent laparoscopic total hysterectomy combined with uterosacral ligament shortening from January 2017 to December 2019 in the Affiliated Hospital of Xuzhou Medical University. Their quality of life and sexual life satisfaction before and six months after operation were evaluated on the basis of their symptoms, pelvic organ prolapse quantitative examination (POP-Q), the MOS 36-item short-from health survey (SF-36), pelvic floor distress inventory-short form 20 (PFDI-20) and pelvic organ prolapse/urinary incontinence sexual function questionnaire 31 (PISQ-31). Results Six months after surgery, patients presented remarkable improvement in each values of POP-Q compared with preoperative levels, except total vaginal length (TVL) (P<0.05). Six months after surgery, their SF-36, PFDI-20 and PISQ-31 scores were significantly improved compared with preoperative levels (P<0.05). Their quality of life and sexual life satisfaction were markedly improved. Conclusions Laparoscopic total hysterectomy combined with uterosacral ligament shortening is effective in treatment of uterine prolapse patients in the perimenopausal period, with improved quality of life and sexual life satisfaction, which is worthy of clinical application and promotion.
