Effectiveness evaluation of cisternostomy in the treatment of severe traumatic brain injury

Abstract
Objective To evaluate the application of cisternostomy in the treatment of severe traumatic brain injury (sTBI). Methods A total of 40 patients with sTBI who were admitted to Department of Neurosurgery, the First People's Hospital of Xuzhou were enrolled. They were randomly divided into two groups (n=20): a subdural drainage group and a cisternostomy drainage group. Both groups were compared for their intracranial pressure at 0 h (at the end of operation), 6 h, 12 h, 1 day, 3 days and 5 days after operation and Glasgow coma score (GCS) after operation (1-14 days). Results The intracranial pressure in the cisternostomy group at each time point was remarkably lower than that in subdural drainage group (P<0.05). The cisternostomy group showed better GCS than the subdural drainage group. Conclusions Cerebrospinal fluid drainage after cisternostomy can effectively relieve obstruction of cerebrospinal fluid circulation, reduce the intracranial pressure, increased postoperative GCS, promote the recovery of neural function and improve prognosis.
