Relationship among neutrophil-to-lymphocyte ratio, serum uric acid and cardiovascular events in patient with maintenance hemodialysis

Abstract
Objective To explore the relationship among neutrophil-to-lymphocyte ratio(NLR), serum uric acid(SUA) and cardiovascular events in patients with maintenance hemodialysis(MHD), and analyze the risk factors for cardiovascular events.Methods A total of 86 MHD patients who were subject to MHD in Blood Purification Center, the Affiliated Hospital of Xuzhou Medical University from August 2020 to August 2021 were enrolled. According to the presence of cardiovascular events, the patients were divided into two groups: a cardiovascular event group(n=28) and a control group(n=58). Both groups were compared for general information, clinical characteristics and laboratory examination results. Multivariate logistic regression analysis was used to screen the risk factors of the incidence of cardiovascular events in MHD patients. A receiver operating curve was plotted to evaluate the prognostic values of NLR and SUA for cardiovascular events in MHD patients.Results Statistical differences were found in the history of diabetes, systolic pressure, intact parathyroid hormone(iPTH), NLR and SUA between the two groups(P<0.05). Multiple logistic regression analysis showed that the levels of NLR and SUA were the independent risk factors for cardiovascular event in MHD patients(P<0.05). The ROC curve showed that the AUC of NLR in predicting cardiovascular events was 0.767, and when the optimal cut-off value was set at 4.7, the sensitivity was 71.4%, and the specificity was 82.8%, respectively. The AUC of SUA was 0.647,and when the optimal cut-off value was set at 344.5 μmol/L,the sensitivity was 64.3%, and the specificity was 63.8%, respectively.The predictive value of NLR combined with SUA was 0.802, and when the optimal cut-off value was 0.316, the sensitivity was 75.0%, and the specificity was 74.0%.Conclusions NLR and SUA are associated with the occurrence of cardiovascular events in MHD patients. The detection of NLR and SUA had certain clinical value in predicting cardiovascular diseases in MHD patients.
